TEAM FEES: ALL TEAMS $375
TEAM MANAGERS INFORMATION
You will be required to bring the following to check-in:
1. ROSTER: Current 2007/2008 Fall Roster that is signed by your local club registrar
2. PLAYER ID CARDS: Current 2007/2008 Player ID Cards. Each player must have a properly signed and laminated USYS Identification card. The ID card must be properly prepared, laminated and contain the proper information to be accepted – name, date of birth, ID number, Team Code and Team Name. The card must also have a recent photo of the individual and be signed by both the Local or Member Association Registrar and the holder of the card. Please place these in alphabetical order for faster check-in. THANK YOU!
3. ADULT PARTICIPATION PASSES/KIDSAFE BADGES: (STYSA Teams Only) Current 2007/2008 Adult Participation Passes for every Coach/Assistant Coach who will be on the sideline in an official capacity for the games. Each coach, assistant coach, trainer or manager wishing to be on the team sideline must have visible their Adult Participation Pass or KidSafe Badge confirming that they have complied with the state requirement for a background check. A maximum of 4 adults (with proper identification) will be allowed on the team sideline during a game.
4. MEDICAL RELEASE FORMS: Medical Release forms ARE NOT REQUIRED!!
